Request Access to FMIS
To request access to FMIS, please complete a FMIS Security Form ( FMIS40 ). See FMIS40in for the instructions. Forward completed form to your Fiscal Officer for approval. All applicants must have a signed General Confidentiality Notice ( UH Form 92 ) on file. The Fiscal Officer indicates on the FMIS-40 that the applicant has signed the General Confidentiality Notice. Submit approved Security Form to
